By the way, I think Britain's National Health system does some stuff like this. They serve everyone in their country for less money (adjusted for population difference) than we spend just on Medicare.
The Dutch doctors still make house calls. About half their practice (for GPs) is house call or other non office patient interaction. My huisarts actually accompanied me to the ER for an exam and hospital admission.
