@4: Black holes can (and presumably do) collide, leading to larger and larger black holes. Also, the supermassive black holes commonly found at the centers of galaxies are a different breed from the stellar mass ones thought to be formed by collapsing stars. It's a bit up in the air how they ended up so big, but the prevailing thought is that they started out as merely large black holes (formed by the collapse of large stars) and then ate very well indeed.
