Watching Bear Grylls do it out there in the desert had lead me to believe it was genuinely dangerous, if not life-threatening. He filtered the crap out of it (figuratively, that is) and even then was adamant that it was high-risk. Last time I take urine drinking advice from him.

@6: I suspect the biggest danger for someone in a survival situation (who presumably is drinking *only* piss, or nearly so) would be the salt content.

Of course, if you distill it (say with a solar still) it's just water...

Somebody forgot to mention gout. Uric acid is bad for people with gout, so that can be an issue.
However, since drinking piss is not my cup of tea, I don't have to worry about that.

He probably should have asked how long you can safely store it and then drink it. Cause that stuff goes bad fast. Best to freeze it if the piss drinker in question is trying for gallons a day. Which only begs another question - if you only drink piss, should you drink 8 cups of piss a day to keep yourself healthy, like with water, or do you have to drink more to balance out the pissyness? Could you survive on piss alone (hydrationwise anyway)? What about with cow urine? I feel like there's gotta be a piss guru in India somewhere, cow urine is commonly taken as a health tonic there.

Unfortunately the medicine in this question is WRONG. Yes you don't want do drink infected piss. However, this nonsense about the person having "horrible" kidneys is stupid. Day to day, the waste removed by a person is equal to the amount generated if the kidney function is stable. Someone with failing kidneys at 50% power will end up with 2 x the concentration of toxins in their blood, BUT, their toxin output in a day is still the same. If someone had acutely failing kidneys that were NOT doing this, then their urine would be LESS toxic, not more. That said, the whole matter depends on the concentration of the urine anyway. There is a lot less toxin PER ml from someone who's been consuming tons of liquid than a dehydrated person (same amount of toxins per day, different amounts per ml). Worst case scenario you drink ALL of someone's piss --> you double your toxin load, which is not unlike losing a kidney, which is.... not a big deal!!

Not everything excreted in the urine is a toxin, it's just waste. Urine contains urea, uric acid, electrolytes, traces of hormones, bit of sugar.
Now the first question is: what happens to these if you ingest them? Electrolytes, sugar and hormones are absorbed in the intestines and get into your blood. In a person with healthy kidneys the excretion of the extra electrolytes is no problem, the sugar will be metabolised normally. I suspect the amounts of hormones are too small to affect the drinker even if s/he drinks the urine of someone of the opposite sex regularly.
Regarding urea and uric acid, I am not sure if they even get absorbed in the intestines. If they get excreted with the faeces: no problem.
If they get absorbed:
- Urea is just the waste product of protein metabolism, that means someone on a meat-rich diet like Atkins' diet already excretes a lot more urea than someone on a carbohydrate-rich diet. So, unless the urine-provider and the urine-drinker are both on Atkins' diet, I think healthy kidneys can handle the doubling of urea to excrete.
- Uric acid, however, is a bit more tricky. Someone prone to gout and/ or kidney stones should probably refrain from drinking loads of urine, since uric acid causes/ exacerbates both conditions. Other persons should drink a lot of water in addition to the urine (and again only if they drink lots of urine regularly) to flush the kidneys and prevent uric acid to form kidney stones.

@17: Actually, losing a kidney is a big deal, especially if you are young. We have two kidneys for a reason and cannot survive on one in perpetuity, which is why in Sweden, we don't use kidneys from live people for transplants because those people who give up a kidney are very likely to need a new one themselves later in life, especially considering how long we tend to live these days.

I agree, the biggest factor is if you drink a lot of water so your urin is transparent like water and has no smell.

In fact, everyone should attain the quality of their urin like that(not yellow) whether you're into piss play or not because it keeps the kidneys healthy for much longer.

According to the above study on urine meth concentration (not metabolites), peak levels were reached 14hrs after users in controlled circumstances were administered 20mg, and were as much as 6000 nanograms per ml of piss (4000 nanogram avg). That works out to about 0.6mg of actual meth ingested for drinking a few sips (3oz / 100ml), or a 6mg dose if one drank an entire bladder full of pee, enough to get one high from drinking piss -- even long after the person who was using had come down and wasn't visibly tweaked.

Or, in non-science terms, OMG.

@18, yeah perhaps someone with gout should watch their urate intake. However, if they have some dilute piss now and then it'd be no more threatening than having all the other foods they have to look out for, like a beer or steak. Nondiabetics should not have ANY glucose in their urine; diabetics would give you at most a urine as sweet as their blood (not that sweet). Hormones? We can purify hormones from pee (premarin = PREgnant MARe urINe) you'd have to drink a LOT of piss. As for toxin/waste, same thing. Water, salt, urea, CO2, urate--all of these things are toxic if you don't excrete them. I guess you can make an exception for the egestion of feces but I don't want my anus sewn shut.

@19, that's not accurate except for "we can't live on one in perpetuity." We can't live on THREE in perpetuity. We all die, but people born with one kidney have relatively normal lives, as do people who lose one. For studied durations, it appears to be safe, and even if not without risk, we do other important things with risks, such as have sex. http://www.ncbi.nlm.nih.gov/pubmed/20215… I'd like to hear what "very likely" to need a transplant is.

@21, there is no data that supports drinking enough water to make your water clear. That whole thing about 8 glasses of water a day is made up and we've evolved a thirst mechanism that tells us when we need water. There is some sugar in the urine of healthy people - not much, but traces. Not only glucose, but depending on diet also sucrose, fructose, leucrose...
Apparently, in some surface rivers and lakes, there are already enough hormones to affect the sex of fish. If diluted urine from women on hormonal contraceptives can do that, what effect could the undiluted urine do?
