1. A frame
2. A declaration of intent

The "frame" lets the viewer know where the "art" begins and ends. It doesn't not have to be a literal frame. The declaration of intent lets the viewer differentiate between framed "art" and a frame propped up against the wall.

So, if you want to consider your tampons as "art" (you would not be the first, btw), pull one out, stick it on the wall and declare "BEHOLD! ART!", and soon too can be told you are worthless, please shut up, my five-year-old could do that, etc.
