According to the (not exactly authoritative) Urban Dictionary, it’s short for “for the win!,” used to express enthusiasm for something, and derived from the game show Hollywood Squares. Lol!
i always thought it was a joke -- a play on the careless typing/texting of 13 year olds or cats. similar to LOL1!11.
as an aside, i truly wish this existed.

For the win (ftw) is often used on 1up.com, the lamest site for video game reviews/previews.
FTW is legitimate. For the win. As in: hooray! If LOL is Internet 101, and IMHO and WTF are Internet 102, then FTW and RTFM are Internet 112: Introductory Topics in Geek Code.

I only used FTW while playing FFXI, an MMORPG.
I've always read FTW as fuck the world.
I always read it as "fuck the world" for some reason. I thought it was some kind of absurdly jubilant nihilist taunt. Now I'm disappointed.
FTW is definately one step up in internets code. Like sayin "the tubes" or usin': teh, !!!11!!!, roflcopter, pwn, pwning, pwnd, l33t, sk1llz, n00b, n3wb, etc.

FTW has meant fuck the world since before the web existed. For evidence, I point you to a movie you might enjoy for all sorts of other reasons, The Decline of Western Civilization (Darby Crash!), wherein Exene Cervenka, all pretty and punky in 1980, explains a homemade FTW tattoo.

FTW (For The Win) is legit on the net. Sometimes you see WTF = FTW, because not only is it a convenient palindrome, but it expresses the recent trend toward bizarre image manips that confuse people (like the shock image lotustits).
@22: Geez, THANK you, I was feeling like a lone fucking dinosaur here for a minute... since FTW means one and only one thing to me: Fuck The World. And yeah, I know more than one homemade FTW tattoo (and have made more than one piece of FTW jewelry).
