If you watch the video carefully, you'll notice the voice is dubbed. The hair appears to be a wig, and at the very end, this "girl's" hand is pretty big for a girl. Also, you can really see the male characteristics in the face, particularly the eyes and forehead.

This is a guy dressed up like a woman, designed to subtly confuse the audience and make them squeamish. It's all part of the GOP playbook.
